SUSCEPTIBILITY OF ANAEROBIC FLORA RECOVERED FROM COMPLICATED INTRAABDOMINAL INFECTIONS TO CLINAFLOXACIN AND IMIPENEM.

Clinafloxacin is a new fluoroquinolone with broad -spectrum activity against gram-negative and -positive bacteria, aerobes and anaerobes. Fifty- two patients were entered into a clinical trial comparing clinafloxacin and imipenem in the treatment of complicated intra-abdominal infections. One hundred twenty-six anaerobic bacteria were isolated from specimens of 44 patients, 125 peritoneal or abscess fluid and one, Bacteroides gracilis from blood. MICs for clinafloxacin and imipenem were determined by the micro broth dilution method. Clinafloxacin inhibited 126 strains of bacteria (100%) at less than/equal 1 µg/ml. Imipenem inhibited 124 strains of bacteria (98.4%) at less than/equal 4 µg/ml. One strain of Bacteroides fragilis had intermediate activity and one stain of Bacteroides thetaiotaomicron was resistant to imipenem. The table indicates MIC ranges for Bacteroides fragilis and the three most common Bacteroides fragilis group species.

Anaerobes were isolated from three subjects who developed post-operative intra-abdominal abscesses, one clinafloxacin and two imipenem. Three Bacteroides fragilis and one Bacterides thetaiotaomicron, which were all susceptible to both clinafloxacin and imipenem, were isolated from the abscesses. Clinafloxacin appears to have excellent in vitro and in vivo activity against the anaerobes associated with complicated intra-abdominal infections and has potential as a single agent for the treatment of these infections.
